Services Coordinators for the Home and Community Based Medicaid Waivers make referrals to ATP for assistive technology and modifications for home and vehicles.
Service Coordinators:
► Referrals are submitted electronically. Please read carefully as you complete the form.
Chrome or Firefox are recommended, using Safari may distort form formatting.
► Please hold off sending in a referral if the consumer or their family is currently experiencing flu like symptoms.
► Due to high referral numbers ATP is seeing increased timelines to complete assessments. The current wait time to schedule assessment for a new referral is 48 days.
